General Office Policies

• Office hours and location
  Our office is open from 8:30 AM to 4:30 PM Monday through Friday. We see patients by appointment only; a referral is not necessary unless mandated by an insurance company.
• Appointments and Cancellations
  We value your time, and scheduling of patients is done in a way to minimize your wait. However, delays cannot always be foreseen and avoided. A sincere attempt is made to adhere to the schedule as much as possible.

We ask that all routine appointments be made in advance. If you find you are unable to keep your appointment, please notify our office 24 hours in advance to avoid a cancellation charge. There will be a $50.00 charge after two consecutive missed appointments. This charge must be paid prior to scheduling any further appointments. (This charge will not be billed to your insurance.)

We require all patients to bring on the initial office visit, all pertinent information regarding your visit. This includes results from tests, including pathology, x-ray, etc and the original x-rays and/or other scans. Also bring a list of ALL medications with dosages that you are currently taking. If any of this information is forgotten, your visit will be rescheduled.
